Student Health Service Laboratory
The Student Health Service (SHS) laboratory offers a wide range of services, including blood collection (phlebotomy) and convenient on-site testing.
The laboratory provides services for students seen by Student Health Service providers as well as laboratory tests ordered by external clinicians.
Many tests performed on site are covered by the Student Health Service segregated fee and are offered at no additional cost to the student.
More comprehensive laboratory testing can be collected at Student Health Service and sent to one of our reference laboratories for analysis.
Costs for laboratory services can be discussed with the patient before ordering. Laboratory services are billed to the student’s account.
External Laboratory Orders for Collection at Student Health Service
For laboratory orders from external clinicians to be collected at Student Health Service:
- The ordering clinician must fax the laboratory order to 715-346-4752
- Or email the order to health.services.office@uwsp.edu
The laboratory order must include:
- Student’s name and date of birth
- The clinician’s name and electronic signature
- A diagnosis code
- Fax information (‘fax to’) so results can be returned to the ordering clinician
