TREATMENT RECORDS ARE CONFIDENTIAL AND SECURED IN LOCKED FILES.  RECORDS ARE NOT SHARED WITH ANY OTHER AGENCY AND ARE NOT RELEASED TO OTHER TREATMENT FACILITIES WITHOUT THE VETERAN’S AUTHORIZATION.

To gain access to VA medical health services, see your local County Veterans Service Officers (CVSO’s).
In Portage County, contact Mike Clements at 715-346-1310.
