Submit a Report

All students, faculty, staff, and guests are strongly encouraged to report crimes, submit concerns, and report alleged student misconduct to our office, regardless of where the incident occurred. If you have questions about which reporting form would be appropriate, contact our office at 715-346-2611.

Anonymous reports are accepted for reports of sexual assault, other forms of interpersonal violence, and reports of bias/hate (report does not contain any personally identifying information about the survivor). Note that individuals identified as a Responsible Employee must submit a complete and thorough report in accordance with Title IX requirements. Submitting a report that does not contain specific information may limit the institution's ability to respond.

General Incident Report / Report a Crime - Use this report form to submit information about incidents of alleged student behavioral misconduct, all crimes as required by the Clery Act, or general informational reports.

Student of Concern Report - Use this report to submit information about overall concerns for students (mental health, physical health, concerning behavior, death in the family, academic struggles, etc.).

Sexual Assault Report - Use this report to submit information about incidents of alleged sexual assault. Note that other forms of interpersonal violence (dating or domestic violence, stalking, or sexual harassment) are reported using the form below. Individuals identified as a Responsible Employee must submit a complete and thorough report in accordance with Title IX requirements. All University employees are required to report sexual assaults involving students.

Interpersonal Violence Report - Use this report to submit information about incidents of alleged interpersonal violence to include dating or domestic violence, stalking, or sexual harassment. Reports of sexual assault should be submitted using that specific report form (above). Individuals identified as a Responsible Employee must submit a complete and thorough report in accordance with Title IX requirements.

Bias Incident / Hate Crime Report - Use this report to submit information about incidents of alleged acts of bias or hate crimes.

Academic Misconduct Report (FOR FACULTY USE ONLY) - Faculty use this report to submit information about cases of student academic misconduct. Currently under revision; please contact our office if you have questions.

Sexual Assault Reported to University Employees (WI § 36.11(22)(c)): "Any person employed at an institution who witnesses a sexual assault on campus or receives a report from a student enrolled in the institution that the student has been sexually assaulted shall report to the Dean of Students of the institution. The Dean of Students shall compile reports for the purpose of disseminating statistical information under par. (a)1.b."
