Students can update their contact information and enter&nbsp;emergency contacts in accesSPoint by clicking on the Profile tile.&nbsp;This information will be used in the event of an emergency that places a student or students at risk (i.e. medical or missing person) requiring the University to contact a person you have listed. Having this information on file&nbsp;will save crucial time if ever the need should arise.&nbsp;<strong>Contact information will be kept confidential following all university policy and applicable state and federal regulations.<\/strong><\/p>\n\n\n\n<p><a href=\"https:\/\/accesspoint.uwsp.edu\/psp\/CSPRD\/?cmd=login\">Update Your Emergency Contact Information and Local or Home Address<\/a><\/p>\n\n\n\n<p>Please contact the Division of Student Affairs (<a href=\"mailto:student.affairs@uwsp.edu\">email <\/a>or call 715-346-2481), with any questions regarding emergency contact information.<\/p>\n<\/div><\/details>\n\n\n\n<details class=\"wp-block-create-block-uwsp-accordion-section\"><summary class=\"accordion-heading\">Gender of Record<\/summary><div class=\"accordion-content\">\n<p>A student&#8217;s gender of record reflects the gender the student identified to the university when the student applied for admission to UWSP. Gender is not included as &#8220;directory information&#8221; and will not be disclosed. \u200b<\/p>\n<\/div><\/details>\n\n\n\n<details class=\"wp-block-create-block-uwsp-accordion-section\"><summary class=\"accordion-heading\">Name of Record<\/summary><div class=\"accordion-content\">\n<ul class=\"wp-block-list\">\n<li>A student&#8217;s &#8220;<strong>name of record<\/strong>&#8221; is defined as the name under which the student was admitted to UWSP. The name of record may be changed by following the name change&nbsp;process below.&nbsp;<\/li>\n\n\n\n<li>A &#8220;<strong>primary<\/strong><strong>&nbsp;name<\/strong>&#8221; is defined as that name verified by a birth certificate, marriage certificate, passport, driver&#8217;s license or court order.<\/li>\n\n\n\n<li>The following lists areas\/documents where&nbsp;<strong>primary<\/strong>&nbsp;name will appear (whether or not you have a preferred name in accesSPoint).\n<ul class=\"wp-block-list\">\n<li>Financial Aid documents<\/li>\n\n\n\n<li>Student bill and other Student Financial documents (1098t)<\/li>\n\n\n\n<li>Official and Unofficial transcripts<\/li>\n\n\n\n<li>Enrollment verifications<\/li>\n\n\n\n<li>Payroll<\/li>\n\n\n\n<li>Immigration documents<\/li>\n\n\n\n<li>Health Services<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>A<strong>&nbsp;&#8220;preferred name&#8221;&nbsp;<\/strong>is a first or middle name change only.&nbsp; This name reflects what a student chooses to be called if different from the primary name.<\/li>\n\n\n\n<li>The following lists areas where&nbsp;<strong>preferred<\/strong>&nbsp;name will appear.\n<ul class=\"wp-block-list\">\n<li>Online Directory<\/li>\n\n\n\n<li>Canvas<\/li>\n\n\n\n<li>Class Rosters<\/li>\n\n\n\n<li>Grade Rosters<\/li>\n\n\n\n<li>Student Services Center (in accesSPoint)<\/li>\n\n\n\n<li>Degree Audit<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/details>\n\n\n\n<details class=\"wp-block-create-block-uwsp-accordion-section\"><summary class=\"accordion-heading\">Social Security Number<\/summary><div class=\"accordion-content\">\n<p>The use of the social security number as part of your university record is convenient for you and the school. It provides you with lifetime continuity of records and with an easily remembered number for requesting transcripts or referring to your permanent records in future years.&nbsp;While submission of your social security number is voluntary, you should consider carefully the advantages of having the social security number as part of your university record.&nbsp;We urge you to include it.&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If you supply your social security number, the university will not, without your written permission, use it for purposes other than routine record keeping and institutional statistics.<\/li>\n\n\n\n<li>Registration materials received without social security numbers will be processed and another form of student identifier number will be assigned.<\/li>\n<\/ul>\n<\/div><\/details>\n<\/div><\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"uwsp-accordion blockspacetop blockspacebottom\"><div class=\"accordion\" id=\"\"><div class=\"card-intro\" style=\"text-align:left\"><h2 class=\"h3\">Change Processes<\/h2><\/div><div class=\"panelcontainer nofirst\">\n<details class=\"wp-block-create-block-uwsp-accordion-section\"><summary class=\"accordion-heading\">Changing First or Middle Name (Preferred Name Change) for Current Students<\/summary><div class=\"accordion-content\">\n<h3 class=\"wp-block-heading\">Process to Change First or Middle&nbsp;Name<\/h3>\n\n\n\n<p>Students enrolled&nbsp;at UWSP&nbsp;may change their first&nbsp;or middle&nbsp;name by submitting&nbsp;a&nbsp;<a href=\"https:\/\/www.uwsp.edu\/office-of-the-registrar\/wp-content\/uploads\/sites\/42\/2026\/03\/Change-of-Preferred-Name-Form.pdf\" data-type=\"attachment\" data-id=\"1305\">Change of Preferred Name Form<\/a>.&nbsp;&nbsp;Requests&nbsp;for&nbsp;last&nbsp;name&nbsp;changes require&nbsp;the&nbsp;Primary Name Change Form&nbsp;and&nbsp;one form of&nbsp;legal&nbsp;documentation.&nbsp; If you are no longer enrolled at UWSP and wish to have your name changed on university records, review Changing Name for Former Students section.&nbsp;&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Effect of Preferred Name Changes on Student Records<\/h3>\n\n\n\n<p>A student&#8217;s preferred name change will be reflected in&nbsp;online&nbsp;directory&nbsp;information, class rosters, grade rosters, Student Services Center in accesSPoint, and the degree audit.&nbsp;&nbsp;&nbsp;Student employment records will not be impacted by name changes.<\/p>\n\n\n\n<p>Please be advised that changing your first name with the university is not the same as legally changing your name&nbsp;through the courts.&nbsp; If you have a legal name change, please complete the Change of Primary Name&nbsp;Form and provide&nbsp;supporting documentation.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">FERPA<\/h3>\n\n\n\n<p>A student&#8217;s updated name of record&nbsp;will be disclosed as &#8220;<a href=\"https:\/\/www.uwsp.edu\/office-of-the-registrar\/ferpa-overview\/\" data-type=\"page\" data-id=\"39\">directory information<\/a>&#8221; unless the student refuses to permit such disclosure.
