IACUC Application Forms

Protocol Forms

  • ​​​IACUC Laboratory Research Protocol Application
    • Complete for research with vertebrate animals in the laboratory.
  • IACUC Field Research Protocol Application
    • Complete for research with vertebrate animals in the field.
  • IACUC Protocol Renewal Form
    • Complete if renewing protocol for the first or second time since triennial review (Due dates are 1 year and 2 years after approval).
    • For projects continuing beyond three years, complete a new Protocol Application for triennial review. (This satisfies year 3 renewal requirements at this time.)
  • Protocol Modification Form
    • ​Used to request changes to previously approved protocols. See Modification Form for list of scenarios where a modification is required.
    • Requests to add major survival surgery to an existing protocol OR requests resulting in major changes to the study objectives of an existing protocol require a new protocol submission.
  • Research Personnel Addition Form
    • Submit to modify research personnel associated with protocol after personnel has completed CITI training & in-person training.
  • Protocol Closure Form
    • ​​​Complete this form when work with animals as part of research has concluded, or PI chooses to close project.

Other Forms

  • ​​IACUC ​Exemption Form
    • Complete this form for Observation Only studies.
  • Unexpected Event Form
    • Submit this form within 72 hours of event.
  • Animal Welfare Concern Form
    • ​Submission of this form initiates the IACUC review of allegations on violations. Violations include: deviation from approved protocol, animal welfare regulations, and/or improper animal care.
    • If submitter wishes to remain anonymous and not use IACUC Document Submission, interoffice or post mail to:
