COVID-Specific Accommodation Request

This accommodation process is intended for students with a documented disability that may have unique situations involving their condition due to the impact of the COVID-19 pandemic. This may include concerns about face covering usage or COVID-19 vaccine intolerance due to allergy or other underlying medical conditions. After collecting appropriate documentation from the student, DATC staff will work with faculty and academic departments to evaluate if online/remote access can be reasonably established. We will consider requests received after August 1, but they may not be fully reviewed prior to the fall semester starting and course delivery options may be limited.

Students interested in housing accommodations due to any pandemic-related situations should also contact Residential Living at 715-346-3511.

Faculty, academic staff, and university staff should contact Human Resources for questions regarding employee face coverings and other COVID-specific accommodations.

The University's COVID-19 resource page contains the most up-to-date information on policies and resources related to the pandemic.

Please complete the accommodation request form below and schedule a meeting with DATC staff to discuss your concerns. Appointments may be reserved via Navigate or by calling the office during regular business hours (715-346-3365).
