Change Form
Use this form if you would like your telephone bill mailed (or e-mailed) to
an address other than the one listed with Registration and Records. The
Telephone Support Office uses the address information listed with
Registration and Records, which is updated daily. If you made the change in
Registration and Records, it will be automatically updated in the Telephone
Support Office within 24 hours.
Authorization Code Application
Use this form if you would like to receive a telephone authorization code.
An authorization code will allow you to place telephone calls from any
telephone on-campus. The 7-digit code will be billed directly to the account
information provided.
Authorization Code Cancellation Form
Fill out this form if your code is lost, stolen, misplaced, or if you simply
want to cancel your code.
Domestic Long Distance Plan Change Form
If you use your UWSP Authorization Code to make off-campus telephone calls,
you can switch long distance plans by filling out this form.
