This form allows you to sign up for UWSP Customized Cellular Service. Are
you in need of cellular service for one month? Two months? This service
allows you to select your contract length. Limited number available. Limited
phone selections available.
Cellular Application - Business Use Only
This form allows you to apply for UWSP Business Cellular Service.
Cellular Feature Changes
This form allows you to add and remove cellular features from your cellular
contract.

This form allows you to switch from one UWSP cellular plan to another.
Cellular Renewal - UWSP Customers Only
If you are currently a UWSP Cellular customer, and have exceeded 20-months,
this form allows you extend your contract (9-months) and purchase a new
phone at the contract price.
Cellular Service Cancellation Form
This form allows you to cancel your UWSP Cellular Service.
