Cellular Forms

This form allows you to sign up for UWSP Cellular Service. This is for personal use only.
 
This form allows you to apply for UWSP Business Cellular Service. 
 
This form allows you to add and remove cellular features from your cellular contract.
 
This form allows you to switch from one UWSP cellular plan to another.
 
If you are currently a UWSP Cellular customer, and have exceeded your 20 or 16-month period, this form allows you extend your contract (9-months) and purchase a new phone at the contract price.
 
This form allows you to cancel your UWSP Cellular Service.
 
Are you having problems with your cellular service? If so, fill out this form, and someone from the Telephone Support Office will respond within 24-hours (business hours).
 
This form allows you to sign up for a data card.  If you are a UWSP Faculty/Staff member who wants to purchase a SmartPhone, please stop by the Telephone Support Office.
