Alumni Registration Form and Instructions
To register: download the Alumni Graduate Participant Follow-Up form (choose Save, not Open). 
Helpful tip:  To make it easier to put your information on the form (in your word processor) before typing, hit the INSERT button on your keyboard once. This will allow you to overwrite what is already there. To turn off this feature, hit INSERT once more.
Complete the form and use one of the following methods to send it:
  1. If you are using Microsoft Word, go to File -> Send To -> Mail Recipient... and Word will automatically attach it to a new email message
  2. Or attach it to a new email message manually
  3. Or copy and paste it into a new email message
