Use this form for registrations to attend conferences, workshops, seminars or events requiring an entry fee.
The Purchasing Card or Travel Card can be used to pay registrations; this form is used most often when a credit card payment is not accepted.
Instructions:
For registration/entry fees over $50 only.
Attach the completed conference/event registration form and documentation that indicates conference specifics such as date, location, fees, etc. and obtain authorized signatures.
Forward request with attachments to the Payment Services Office, 041 East, Main. The conference/event registration form will be sent with the payment.
