Receipts attached to forms
Always submit the original receipt, copies are not acceptable.
A duplicate copy of a check from a check register is NOT adequate proof of payment.
Please do NOT use a highlighter or place tape over printing on a receipt; many receipts use thermal paper that result in the printing on the receipt DISAPPEARING when a highlighter or tape is used.
