Authorization Code Application - Use this form to obtain a 7-digit authorization code. It will allow you to place telephone calls from any telephone on-campus. The 7-digit code will be billed directly to the account information provided.

Long Distance Plan Change Form - If you use UWSP telephone service, use this form to switch long distance plans.

Fill this form out to contract for UWSP business cellular service. This service requires a 6-digit department account and authorization.
Cellular Feature Change - Personal Use Only
Use this form to add and remove features from your personal cellular contract.
Cellular Plan Change - Personal Use Only
Use this form to switch from one cellular plan to another on your personal cellular contract.
Cellular Renewal - Personal Use Only
If you are currently a UWSP customer, and have reached your renewal term, this form allows you to extend your contract and purchase a new phone at the contract price.
