Charges/PaymentsCHARGESInitial Evaluation & AssessmentThere are no fees associated with the initial evaluations that are performed in the UWSP Sports Medicine Clinic. RehabilitationFees are charged for all rehabilitation services provided in the UWSP Sports Medicine Clinic. However, payment is not required at the time of service. Patients or their insurance companies will be billed for incurred fees following their visit. OVERVIEW OF BILLING AND PAYMENT PROCESS for StudentsAt the beginning of your visit to UWSP Sports Medicine Clinic, you will be given a carbon-copy encounter billing form that will stay with you until the end of your visit. This form will list the various services rendered. At the end of your visit you must stop at the check-out desk where one of the staff assistants will total your charges, check the form for completeness and answer any questions Before you leave, you will receive a copy of your bill. This can be used for submission to your insurance company with all of the information most insurance companies request (date of service, type of service, coded description, diagnosis, and the price). Keep copies of all receipts. The Bursar's Office will mail out statements to your permanent address until the bill is paid. This statement includes the date the bill was posted to your account, the words " Rehabilitation Center ," and the amount of the charge. Charges posted to the student account must be paid at the Bursar's Office, UWSP Bursar's Office, 2100 Main Street , Stevens Point , WI 54481 . HOW TO FILE A MEDICAL CLAIM WITH YOUR OWN INSURANCE COMPANYIf your health insurance company provides coverage for care provided at UWSP Sports Medicine Clinic, it is your responsibility to know the limitations and requirements of your plan prior to receiving services at UWSP Sports Medicine Clinic. In many cases, your insurance plan may require pre-authorization and possibly an out-of-network referral in order for the service to be covered. If you are unsure of your coverage, please contact the customer service department of your insurance plan. Most companies list their phone number on the back of the insurance card. Make sure you keep your copy of the UWSP Sports Medicine Clinic bill for submission to your insurance company - don't throw it out, as there is a fee for additional copies. In order to submit a claim you must fill out your insurance company's claim form and send it along with the copy of the bill (yellow) to your insurance company. Keep a copy of the bill for your records. Sometimes, insurance companies have follow-up questions that are easily and quickly answered by us and may prevent a delay in your reimbursement. For this reason, you will be asked to sign an authorization to allow UWSP Sports Medicine Clinic to release medical information to your insurance company. UWSP Sports Medicine Clinic cannot accept the responsibility for submitting or collecting your insurance claim or for negotiating a settlement on a disputed claim. |
