New Injury Evaluations

If you have have suffered an injury and would like the staff at UWSP Sports Medicine Clinic to evaluate that injury, walk in appointments are available.

Rehabilitation of Injuries

If you have an injury that needs to be rehabilitated at the UWSP Sports Medicine Clinic, you can do so by making an appointment.

You can make appointments by calling 346-4570 or 346-4772.

You can also make appointments by e-mailing Rehabilitation.Center@uwsp.edu with appointments in the subject box. Your appointment will be confirmed when you receive a reply from the Rehabilitation.Center@uwsp.edu.

Items To Bring With You

If you are coming to the UWSP Sports Medicine Clinic for the first time to have an injury evaluated you will be required to bring the following:

• Notice of Privacy Practices (Click on to print off)
• Patient Registration Form (Click on to print off)
• Student or Faculty/Staff ID
• Shorts (if a lower extremity or back injury)

If you are coming to the UWSP Sports Medicine Clinic for the first time for rehabilitation you will be required to bring the following:

• Notice of Privacy Practices (Click on to print off)
• Patient Registration Form (Click on to print off)
• Prescription from your physician specifically stating your need for rehabilitation with an athletic trainer or physical therapist
• Insurance card
• Student or Faculty/Staff ID
• Clothing Suited for Physical Activity
