Forms and Handouts

Your FAFSA usually tells us everything we need to know to create your financial aid offer. But sometimes we need supplemental information to complete the process. Forms requested by the Financial Aid and Veteran Services office will be listed on in your Task Tile in accesSPoint.

Below you'll find some of our informational brochures and forms.

Invest In Yourself Brochure: Cost is a major consideration when selecting a college. UWSP offers a variety of assistance programs to help bridge the gap between the Cost Of Attendance (COA), and your family's financial resources. Start here to learn more about how to apply for financial aid, the different types of aid programs, UWSP's COA budgets, and more.

Six Items and 22 Minutes: Filing the FAFSA isn't as complicated as it seems - you only need six items and 22 minutes! View this document for a quick breakdown of what's needed to get started.

Financial Aid Checklist: Do you want to make sure you have everything completed in order to receive your financial aid? View our financial aid checklist for helpful tips.

FAFSA Completion Tips: Keep these tips in your back pocket! This handout provides information before applying, while applying, and after applying for the FAFSA.

Financial Aid Vocabulary Cheat Sheet: Student financial aid has a lot of acronyms and terms that can be confusing. Use this glossary to take some of the confusion out of the terms to help you understand more about your financial aid.

Citizenship Affidavit Form: Sometimes the Social Security Administration is unable to confirm you are a U.S. citizen. Submit this form with the requested documentation to the Office of Financial Aid and Veteran Services in order to be considered for student financial aid.

Scholarship Reporting Form: Congratulations, you were selected to receive a scholarship! Unfortunately, scholarships count as financial aid and the determination of the other aid offered. Be sure to notify us each year of any scholarships that are not already on your financial aid offer.

Sample Financial Aid Offer Letter: View a sample of our financial aid offer letter that is sent to first-year students in the mail. These disclosures also are sent in the financial aid offer packet.

Financial Aid Appeal: Does the information on your FAFSA not accurately reflect your current situation? View our Job Loss and the FAFSA handout. You may consider completing our financial aid appeal form to see if your aid can be adjusted based on your situation. For more information on whether your situation is appealable, visit our financial aid appeal web page

SAP Appeal Form: Complete this form to appeal your Satisfactory Academic Progress (SAP) in order to be eligible to receive financial aid.

SAP Documentation for Appeal: Not sure what kind of documentation is needed for your Satisfactory Academic Progress (SAP) appeal? View this document for a list of examples based on your situation.
