Vehicle Use and Driver Authorization

The use of State-owned vehicles is for Official State Business. If an employee wishes to travel with his or her family, or combine business with personal travel, that employee should use his or her own vehicle.

All fleet vehicle users (employees) are required to have a Vehicle Use Agreement form on file with the Transportation Office. If your department has students driving State vehicles, they are required to have a Student Driver Authorization form on file. The student form must be signed by the supervisor or advisor who is requesting that the student drive. The Student Driver Authorization from expires each year on May 31st.

A Notarized Statement is required for: 1. Individuals holding out-of-state or foreign driver's licenses. 2. Individuals having Wisconsin License for less than one year, due to previously being licensed in another state/country. (Statement must list any moving violations and/or accidents in the past two years.)

State policy requires that driving records be checked annually. Use of the State vehicles may be denied if an individual's driving record shows multiple accidents and/or violations. Driving records are confidential and will be handled as such. If you have a question regarding your own driving record, please call the Transportation Office at 715-346-2884.

Student Organizations

A Student Organization State Vehicle Request Form must be filled out by the student organization in order for that organization to reserve a State vehicle. This form must be in the Transportation Office before the vehicle can be released. The form must be filled out completely and be signed by the requester, the requester's advisor and the budget director of the Student Government Association if the organization receives SGA funding for the trip.
