UWSP Graduate Program Application Materials Check Off List

In the past, some application files did not progress through the final review process because they were incomplete. All items listed below are required. It is your responsibility to meet the deadlines.

colleges, colleges, and universities you have
attended (except UW-Stevens Point).
Verify with your Registration/Records Department that your Fall semester grades will be included on your transcript.

- Statement of Professional Intent including why you want to enter the profession, and why you think you will be successful. State your professional interests and goals. PLEASE SIGN THIS LETTER. ____________ ____________
- Clinical Experience Summary form ____________ ____________
- Three (3) Letters of Recommendation . At least ONE (1) letter from an instructor and ONE (1) from a clinical supervisor in your program. The letters should address potential for success in graduate school, and past academic and clinical experience. ____________ ____________

Please note that the application materials in Section I must be completed and received by the Admissions Office no later than January 14, 2009. The application materials in Section II must be completed and received by the School of Communicative Disorders by January 14, 2009.
