UWSP Graduate Program Application Materials Check Off List
In the past, some application files did not progress through the
final review process because they were incomplete. All items listed
below are required. It is your responsibility to meet the deadlines.
I. Must be received by Monday, January 14. 2013.

- Admission to Graduate Study form and/or online application
- Application fee $56.00 (non-refundable)
- Official transcripts from ALL Technical
Colleges, Colleges, and Universities you have
attended (except UW-Stevens Point)
Verify with your Registration/Records Department that your fall semester grades will be included on your transcript
II. Must be received by Monday, January 14, 2013

- School of Communicative Disorders Application form
- A listing of ALL courses enrolled in during your final semester. Put your name on this list.
- GRE Scores (University - 1919; Audiology - 0602; Speech-Language Pathology - 0620
- Statement of Professional Intent including why you want to enter the
profession, and why you think you will be successful. State your
professional interests and goals. PLEASE SIGN THIS LETTER.
- Clinical Experience Summary form
- Three (3) Letters of Recommendation . At least ONE (1) letter from an
instructor and ONE (1) from a clinical supervisor in your program. The
letters should address potential for success in graduate school, and
past academic and clinical experience.
- Assistantship Application (optional)
Please note that the application materials in Section I must be
completed and received by the Admissions Office no later than Monday, January 14, 2013. The application materials in Section II must be
completed and received by the School of Communicative Disorders by Monday, January 14, 2013.
