APPLICATION

Apply early!  Applications are accepted in the order in which they are received, and enrollment is limited.  Some classes may fill by early Spring.  Incomplete applications will be returned, as will applications received after the programs are full. For student enrollment, be sure to include your child’s current working and polished pieces.  You may notify us of working and polished piece changes only until July 7th.

PAYMENTS

If paying by check or money order, a minimum payment of the $100.00 non-refundable registration fee is due with your completed application.  Applications received without the registration fee will be returned.  Credit cards are accepted for payment in full at time of application only.
 
A $25 late fee will be assessed for all applications and payments postmarked after July 7th, and to all accounts that are not paid in full by that date. 
 
Faxed applications are only accepted if paid in full with a credit card.

REFUND POLICY

A refund of all fees paid, excluding the $100 registration fee, will be given only if notice of cancellation is received in writing by July 7th.  Cancellation notices may be sent via email to suzuki@uwsp.edu.
