Program of Study
This form constitutes a contract between the student and the graduate committee for the duration of the graduation program. It must be completed and submitted to the Graduate Coordinator prior to reaching 15 credits.
Any changes made to the proposed coursework or research topic must be approved in writing using the Program of Study change form.
Program of Study change form
This form is used when you have a change in your coursework. Sometimes a class is not offered when needed or additional supplementary courses are needed after the original program of study has been submitted.
Graduate Office Space form
This applications needs to be filled out, signed by your adviser and submitted to the Graduate Coordinator the first week of each semester.
Students are able to use their allotted office space for the four semesters they are actively taking courses.
Transfer Credit form
This form is for use in transferring credits from other Universities for use in your graduate degree requirements here at UWSP.
Approval is needed from your adviser and the Associate Dean.
Extension of Time form
If you are unable to finish your degree requirements within the seven year time period you can file for an extension of time. This must be approved the semester prior to your seven year deadline.
Approval is needed from your graduate committee, the Associate Dean and the UWSP Graduate Council.
