All the forms on this site are either in .doc (Microsoft Word) or .pdf (Adobe Acrobat) format. If you need a form that is not available on the website, please stop by the SIEO and we'll be happy to provide it.
Key Forms for Student Organizations
On-Campus Sales – This form is necessary for any operations of a concession or sale of goods on campus by any student group. Sales of Food must include the Food Waiver Form.
Permit to Serve Alcoholic Beverages – Form must be completed and returned to Centers Reservations no later than one week prior to any on-campus event where alcohol will be served.
Work Order Form - Use this online form to request a work order for your student organization office.
Advisor Contract Form - Form for new Student Organization advisors .
Travel Forms
Please review the Student Organization Outdoor Recreation/Travel Policy and complete the travel checklist before you make your travel arrangements.
Hold Harmless Form – This form needs to be completed by every participant in a student organization activity when it is considered to be Outdoor Recreation or Travel.
Emergency Information - Your organization must Provide emergency phone numbers for people on the trip and also obtain information on how to reach people at the university during the trip (i.e., Transportation, Advisor, SIEO staff) in case of emergency.
Promotional Forms
Table Tent Reservation Form - Any recognized student organization or University department can reserve one of the three panels on the six weekly table tents to advertise or promote events/programs directly related to the organization.
Button Maker Rental Agreement - The button maker may only be reserved by a UWSP recognized student organization or an office of department of UWSP.
Instructions for Use:
- Fill out on-line.
- Print out.
- Turn in to Student Involvement and Employment Office (050 DUC).
When finished with the form, it will ask you if you want to save changes. You may save a copy to your computer for future use; if you do not want a copy, click no.
