Application Instructions: Use the
attached checklist as a guide to make sure you have completed all the required
materials.

1. Complete
the Application. Use the Narrative Form provided
(last page of the application) for your narrative (200 words or less). Remember, the committee will be reviewing
your application and narrative, so be sure to proofread! Double space the narrative and justify the
text. To justify text: Use the Formatting toolbar to justify your text
quickly. Select the text and click Justify. (To display the Formatting
toolbar, point to Toolbars on the View menu, and then click Formatting.).
Check your pages and make sure you do
not have a signature line or just one sentence as a page. You may have to adjust your pages!
BE SURE TO PROOFREAD ALL THE FORMS BEFORE YOU SUBMIT
THEM!
2. Review the list of
affiliated hospitals and complete the
hospital choice form. Be
sure to sign the form (type your name), and return it electronically along with your application.
3. Only
list the people you are going to use as a reference. Do not contact them at this time. You will contact them for a reference when
you are applying for a clinical site in the fall.
4. Complete
the Personal Data Form for award
nominations. Be sure to sign the form (type
your name), and return it electronically along with your application.

5. Save
all forms as Microsoft Word Documents using the following format: Your last name and the abbreviation listed below. |

7. Personal interviews with the UWSP Clinical Laboratory
Science Executive Committee will be held Tuesday, February 26th. BE SURE TO SIGN UP FOR AN INTERVIEW
TIME. THE SIGN UP SHEET IS POSTED ON THE
BULLETIN BOARD OUTSIDE OF D127 SCI BLDG.
8. Upon formal acceptance to the Clinical Laboratory Science
Program, the applicant is deemed eligible for his/her Clinical Practicum. This does not, however, guarantee placement
for the clinical practicum.
9. Students are required to tour affiliated hospital
laboratories in

university, or post-secondary school, please submit a copy/copies of the
