Intercollegiate Athletics Student-Athlete Medical Forms

All athletes who plan on participating (on the roster or trying out) must have completed medical form on file in the athletic training facility prior to engaging in practice or contests.

Incoming Freshmen/Transfer Students

You must complete the following prior to participation in intercollegiate athletics:

Physical Examination Form
UWSP Student-Athlete Insurance Form
UWSP Hold Harmless (to be complete at eligibility meeting)
HIPPA Release Form (to be completed at eligibility meeting)

Returning UWSP Athletes (and Student-Athletes who have not participated for 1 year must complete a new Physical Examination)

You must complete the following prior to participation in intercollegiate athletics:

Annual Health Questionnaire
UWSP Student-Athlete Insurance Form (only if insurance info has changed in the last year)
UWSP Hold Harmless (to be complete at eligibility meeting)
HIPPA Release Form (to be completed at eligibility meeting)
