The following list of documents is provided for your information. If you wish to apply to our graduate program simply print out and fill in all of the documents listed in the Graduate School Application Forms section. Send the completed documents to:

Graduate School Application Forms:  Please note that you must submit an application to be accepted into the UWSP Graduate Program and a second application to be accepted into the School of Communicative Disorders Graduate Program.

Please be advised that the School of Communicative Disorders will need a transcript that includes your Fall grades.
